I'm sure someone can better answer your question, as mine is just a partial answer:
Once, I did change companies and Company B had insurance with the same insurance company as my previous Company A.
I had NOT maxed out the insurance, as you ask, though. Anyway, I got a new card, new number, and all the deductible amounts I'd paid prior were zeroed out. I started from scratch as though I had never been insured through them.
Also, not mentioned is what happens if this insurance has a "pre-existing condition" clause where you have to wait for a year or whatever, before the condition is covered on your new policy. I've never been in one of these plans, but this is an interesting thing to consider as wel if one had a pre-existing condition that was covered originally, but what about now?
Also, states have different rules on how insurance companies operate, so this probably has to be considered, too.
So, while I may suspect what the answer is, I can not say for certain.

It will depend on what company you're with and applicable state laws. The insurance company I work for will carry over deductibles paid, etc if you switch from one company to another in that calendar year. As to if that's true for the company you're going through, hard to say without seeing their policy.
