Or is it clean as a whistle? I'd assume the former, because human bodies are pus-pits that accumulate the nastiest shit wherever and whenever they can.

You can get irritants behind the eyeball, and this is especially true in some environments, like asbestos plants, most mine and drilling operations, etc. It's a real problem when an abcess forms behind the eyeball, but not that unusual.
