One of the essential differences is that in a fair one you take the ball, in an unfair one you don’t. It is more complex than that though. One example if going “over the ball” or with the foot up and studs showing. Those can very well earn you a straight red, whether you take the ball or not.

Maybe. I always wonder how people can be so certain about dives, though. If his heels were clipped, it wasn’t a dive, nor necessarily a foul.
Perhaps I’m just coloured by years of seeing people dive. I really haven’t noticed anything particularly bad by the Americans. They still seem more honest than other teams IMHO.
