Are cancer rates increasing?

My doctor claims that what is causing the increase in deaths from cancer is the decline of smoking! Hey, how can that be? Used to be that most of his patients died from heart attacks and related things. With the decline of smoking (and, oh yes, the increasing use of statins) they are now mostly dying from cancer.

It is not generally appreciated that tobacco is much likelier to kill you from heart disease than cancer. It was just that the statistical link between smoking and lung cancer was so strong (something like 90% of lung cancer cases were among smokers) that that was the first big strike against tobacco.
