# Can anyone recommend a good template for email in forms?

I am a total doofus when it comes to html etc editing. I am trying to create an online application form for my dog rescue, I am failing at it.

I have frontpage, and I tried creating my own form…I thought I was doing okay, but it wouldn’t work (wouldn’t send in the form).

I’ve tried response-o-matic, but I can’t seem to edit it…and I am not sure if it would work anyway, because it is just html code that I add to the site, so how is that different from what I did in frontpage.

I am looking for something that I can fill out on a website, and have them host the page, if that makes any sense (like a guestbook).

I’m pretty clueless here, so I am hoping someone will feel sorry for me, and recommend something I can use (or tell me what I am doing wrong 🙂 )

Just plug in your information and it will generate the php script for you. It’s also editable if you want some different field, etc.

FrontPage forms will only work if your service provider has the FrontPage service extensions operating. Many don’t, mostly because it’s Microsoft.
