**[A plea from doctors in Italy: To avoid Covid-19 disaster, treat more patients at home](https://www.statnews.com/2020/03/21/coronavirus-plea-from-italy-treat-patients-at-home/)**

> The experience of the Bergamo doctors is crucial for U.S. physicians to understand “because some of the mistakes that happened in Italy can happen here,” said Maurizo Cereda, co-director of the surgical ICU at Penn Medicine and a co-author of the paper. . . .
>
> . . . hospitals might be “the main” source of Covid-19 transmission, the Bergamo doctors warned. The related coronavirus illness MERS also has high transmission rates within hospitals, as did SARS during its 2003 epidemic.
>
> Major hospitals such as Bergamo’s “are themselves becoming sources of [coronavirus] infection,” Cereda said, with Covid-19 patients indirectly transmitting infections to non-Covid-19 patients. Ambulances and infected personnel, especially those without symptoms, carry the contagion both to other patients and back into the community.
>
> “All my friends in Italy tell me the same thing,” Cereda said. “[Covid-19] patients started arriving and the rate of infection in other patients soared. That is one thing that probably led to the current disaster.”

Patients with mild to moderate symptoms need to be cared for with house calls, and then, if severe, transported to designated fever hospitals by dedicated fever ambulance crews. Otherwise it sounds like the social distancing won’t work.

In Oregon, there are boxes at the drive-up points. You can also mail your ballot. You can also wait for election days and hand it to the volunteer at a collection point. Voting by mail just isn’t difficult.

I know that “wild speculation” isn’t breaking news. But if the USA suspends exports of critical medical supplies, as some other countries have done, and as there is some speculation might happen … then it will have seismic impact on Australia, similar to declaration of war. If the medical supply contracts are torn up, the international patent agreements protecting those suppliers will be torn up too.

Some very sobering numbers coming out of Italy. The death toll has risen by almost 800. 13 doctors have died.
Also, I saw this stat on Twitter: Total is now over 300K

It took 3 months for the first 100K
It took 12 days for the second 100K
It tool 3 days for the third 100K

Well yeah. The Dems should make it clear that the alternative is that they will impeach Trump again, this time specifically over his continuing dereliction of duty in the face of the pandemic.

The GOP Senators can decide whether they want to vote to keep a devastatingly incompetent President in office at a time when that incompetence might result in a million or more deaths.

The memo to the GOP would be: either use the 25th Amendment to put Pence in charge for the duration, or we’ll hang this catastrophe around the necks of every GOP Senator.

The GOP would still behave just as they have before. But as the bodies piled up, it would cost them.

According to the John’s hopkins site, the US is now #3 in the world in total infections, behind China and Italy. I expect the US to be in the #1 spot in 2 or 3 weeks. Unless they stop all testing (or reporting on tests) because it “makes them look bad”. They may want to follow the North Korea or Russia example (deny a problem with the virus, and then in 8 months, announce a tragic mining disaster that kills 4.5 million people by accident.)

It was reasonable two days ago, but both [science](https://boards.straightdope.com/sdmb/showpost.php?p=22202797&postcount=1240) and the extent of the spread has outdated some of this.

And even by two-days-ago standards, he doesn’t understand South Korea, where less than one percent of the population has been tested. South Koreans do not rush out to get a test whenever they have a bad cold, because they know there aren’t enough tests for that. Just as they don’t rush out to see a health care provider.

If your cold turns into something so bad that your doc tells you, on the phone, that you need to go to a hospital, you need to go to a hospital pavillion where you will be totally isolated from non-fever patients by being in a separate building. Testing everyone who might have COVID-19 will not change the treatment, and isn’t realistic for several months yet. Not even in South Korea.

Universal testing is realistic in Iceland, but, as far as I can tell, no other country.

308,231 confirmed infection
13,067 dead
95,826 recovered

In the US:

26,863 confirmed infection
348 dead
178 recovered

Yesterday’s numbers for comparison:

> 91,952 recovered
>
> In the US:

> 264 dead
> 147 recovered

The US did indeed leap ahead of Iran, Germany and Spain today to take the #3 spot for confirmed infections. At this rate, we will likely take pass Italy on Monday and take the top spot from China on Wednesday.

At this point IMO there is a very real possibility that the US will suffer by far the highest death toll of any nation, like 30-40x what China has tallied. American society is woefully unprepared for the discipline this will take to get thru, and the federal elected leadership is faltering badly in every way (again: IMO).

Worldwide there are now over 1 billion people on lockdown/quarantine. If some jackass takes out the internet, society will collapse overnight. ETA: Not joking.

Yep, we’re heading there, and “exponential growth” is a term that will be regarded with dread for decades to come after being hammered in the hard way in American minds this year. This is no virus to play loosey-goosey with. It only gets stopped with draconian measures, which we don’t/can’t really take.

But as mentioned earlier, I am really puzzled as to how \*\*India \*\*has successfully kept its infections to a tiny fraction of the USA’s. Many regions of India practice poor sanitation, many Hindus like to gather in massive crowds, the healthcare is substandard, the education levels aren’t as high as the West’s, many people lack even a flushing toilet - and yet they are only at a few hundred cases with a few deaths. I would have expected them to have shot to No. 2 or No. 3 worldwide by now, and outrun even the Italians by a big margin death-wise.

Either they are keeping it under wraps by not testing (and the truth will eventually burn its way out,) or the authorities in India are doing a spectacular job.
