Jackie Chan has always been injured during routine stunts. In dangerous stunts, he’s taken proper safeguards.

In Police Story 3, he almost died in a stunt where he was riding a bicycle, crashed into a tree, and then fell out of the tree (less than 1 meter.) He landed on a small stone and it punched a hole in his skull, which is still there today. He let Jay Leno feel his brain on the Tonight Show once.

In Rumble in the Bronx, he jumped about 1 meter onto a hovercraft. He didn’t know the hovercraft had a frictionless surface, he slipped, and broke his ankle. In later scenes, they painted the cast to look like his sneaker, and he finished the movie.

In Police Story 1, he received 3rd degree burns on his hands when he slid down 3 levels of a mall on a pole. He didn’t know the pole’s lights would break, giving him electric burns.

Here’s one that fits:

Jackson “Rock” Pinckney, who played one of Fender’s pirates, lost his eye during filming when Jean-Claude Van Damme accidentally struck his eye with a prop knife. Pinckney sued Van Damme in a North Carolina court and was awarded $485,000. The scene where he lost his eye allegedly made it into the final cut of the movie.

In the forward for William Hobbs’ Fight Direction for Stage and Screen Lawrence Olivier list the litany of his combat accidents including an arrow shot between the lower leg bones on one leg. I personally have a chipped jawbone from one rehearsal accident and been stabbed in the chest.
