A few times, but not in a while. My wife and I pretty much refuse to make small talk w/ cashiers, so as not to be distracted from that.
We almost always get cash back at the grocery - get cash that way WAY more than at ATMs. Usually $100 at a time.
Used to be great when Discover gave you cash back on your cash! 
Yes. Luckily for small amounts that I could afford to lose.
Of course, I was halfway home before I realized it and by then my cash was long gone.
The weirder thing was the day I went through the drive-through window without picking up my food. (I was very distracted that day. I ordered, paid, and then drove away - realizing a few blocks later that I didn’t have any fries. They were still holding the bag at the window when I got back.)
