Psychologist here seconding the suggestion to get him to a neurologist or neuropsychologist right away. When I treat adults who had OCD or tics as kids, they tell me that what anyone would observe was just the tip of the iceberg. I had someone describe her nonstop cognitive compulsive rituals (counting, undoing, etc.) as her “own private hell”. And even though kids are tormented by these things, they often don’t have the conceptual language to adequately describe what they’re going through to someone who doesn’t know what to listen for. Can’t hurt, that’s for sure.
