On the last episode of MEDIUM that I watched, the cop has a brother who is a rapist. The bad brother convinces the good brother that he’s going straight, but Alison fingers him for the rape of a 15 year old. She has also encountered a vigilante nutjob who is killing people on the sex registry.
So the killer goes after the cop’s brother. And the cop watches her walk into his brother’s apartment and tells Alison on the phone, “Sorry, he’s dead. We were too late to save him, but we caught her leaving the building.”
Also on ER, Mark Greene watched a patient (wasn’t it Forrest Whittaker?) die in an elevator without helping him, because the patient was a murderer who had gone crazy after his son was taken by social services and he blamed Mark and had menaced Elizabeth and Ella. Mark discharged the paddles to make it look as if he’d given treatment, but he really just stood there.

The guy Mark let die was named Clauson. He went on a shooting spree but never made it to menace Elizabeth and Ella because they went out for a walk. The police got to the house before they got back, and caught Clauson there.
Forest Whitaker was several seasons later, he lost a malpractice suit against Luka Kovac and took him hostage and was threatening to either shoot him or make him jump off of the roof of his apartment building, I wasn’t quite sure which. A police sniper shot and killed him.
Speaking of characters doing out of character things, in one of his early episodes, Luka barehandedly killed a mugger who attacked him and Abby while they were on their first date. He pounded the man’s head against the concrete until he had a brain hemorrhage.
