There’s no simple answer to that question. The most effective one is whichever one relieves the patient’s symptoms adequately with the fewest side effects that the patient is also willing/able to take as directed. Lots of various effective meds fail to meet those particular qualifications for individual patients.

Nothing more to add except +whichever one the insurance pay for or will authorize or is in their formulary. If the person is self-pay then it might need to be one of the older first or second generation anti-psychotics. An insurance company may require trying and failing with one or two first gen anti-psychotics before they will pay for a newer second generation one.

You have to watch the cytochrome P 450’s with many of the neuroleptics.
[2D-6 in particular](https://testdirectory.questdiagnostics.com/test/test-detail/10490/cytochrome-p450-2d6-genotype?cc=MASTER) can cause a lot of trouble

Just as an aside …

It’s also prudent to avoid grapefruit and grapefruit juice when you’re taking such a medication:

> The effect was first discovered accidentally in 1991, when a test of drug interactions with alcohol used grapefruit juice to hide the taste of the ethanol. A 2005 medical review advised patients to avoid all...
