The play does have to start between the hashes, but all the other players could be outside the hashes. Like well off. The center could be between the hashes with the QB and you could have two separate groups of players at each set of numbers left and right. Sometimes you even see something like that on gimmick plays.

The other main restriction is that at least 7 players, including at least 2 eligible receivers are at the line of scrimmage. While there’s no explicit rule to the effect, practically speaking, the center has to the snap the ball backwards (vs sideways or something), so there’s never going to be more than 10 players at the line on a normal offensive snap.
