Also, does “top” and “bottom” refer to literal positions, or rather who’s doing the penetratiing and who’s getting penetrated?

One doesn’t have to be gay to enjoy a little self play with the nether regions. While I dont know about orgasm, the feeling is most definitely pleasurable.
As for the gay thing, I always thought there was a turn-taking process involved for those couples in which both parties had a desire to ‘cum.’

The latter. You can be physically on top, but if you’re getting it up your… well, you’re the bottom.

In “vanilla” sex, it has to do with penetration, but in the world of BDSM, it has to do with many things that don’t necessarily involve penetration at all; it can describe differences that are as mental and emotional as physical.
But it never has to do with literal positions.

Of course, but if you can wiggle your toes, walk, kick, crawl hands-and-knees, or experience sexual pleasure from stimulation below the waist, you have that ganglion (just not the male gland it’s adjacent to in males).
