One of the minor characters in I think the first season was a black surgeon nicknamed “spearchucker”. Googling, apparently he threw javelin in high school or college, but still, it’s not good.

He was in the movie too. The official story is that he was dropped after the first season because they found out that there were no black surgeons in MASH units in Korea (no word on whether there were psychic corporals).

I’m guessing that even at the time the book was published (1968) and the film was made (1970), “spearchucker” was already recognized to be a racist epithet. My interpretation is that the characters referred to their colleague with the epithet as a form of particularly (and deliberately) obnoxious trash talking. Not a true attempt to put him in an inferior position, such as calling him “boy”.

I’d guess the character was dropped from the series because they didn’t have much for him to do, and the producers were cutting costs.

I’d say the better explanation would be that Kira is rolling her eyes.

No, I think the actor is rolling her eyes at the ridiculousness of the scene. There’s a similar scene in ST Voyager where Janeway gets stuck in some silly black and white holoprogram of Paris’s. Kate Mulgrew is also rolling her eyes.
