Things that someone can do 99 times with no problem, and then the 100th time they get killed? Drunk driving seems like an obvious example, but what else? ---

Russian roulette. Drunk chain sawing. Using a deep fryer. Feeding a bear or buffalo. There’s a metric crapload of stuff you can do once, but won’t survive 100 attempts unless you’re real lucky. And will _never_ survive 200 attempts. Swimming with sharks and no cage? Pullin’ on Superman’s cape? Over 50 years of driving I have a _lot_ more than 100 drunk driving events. Never got stopped or dinged anything. But most of that was long ago and I’m a lot more circumspect these days. That’s a loser’s game these days between advancing age and changing mores. ---

The 1988 story of the skydiving instructor who **forgot to wear his parachute** was re-trending a few weeks ago. It was his third jump of the day and he probably wasn’t expecting the hazard. ---

Working in enclosed areas (think large storm drain or under-street vault) and skipping any of the safety procedures. Working in a trench deeper than 4 feet . . . make that 3 feet. Even with shoring, collapses happen. ---

I _vaguely_ remember an incident in Connecticut in which sewer gases killed a couple of telephone company employees in an underground vault. I can’t find any articles on that incident, but here’s a [Washington Post article](https://www.washingtonpost.com/news/morning-mix/wp/2017/01/18/three-utility-workers-descend-to-their-deaths-in-florida-manhole-overcome-by-fumes/) about three dead utility company employees in Florida. ---
