I guess that ‘traveling plastic surgeon’ thing might be at least somewhat common. My dermatologist in Pennsylvania is one in a group of five or six offices scattered around this rural area, each run by a Nurse Practitioner. They handle routine jobs, biopsies, etc., in the office and all refer surgeries to a plastic surgeon who comes in from New Jersey once a month or so to one of the bigger offices that has set up a surgery center.

That’s true for specialists in general. For example, the city where I live doesn’t have enough children with heart disease to support a full-time pediatric cardiologist, but one comes here a couple times a month to see local children, and when I was a kid in the 1970s, my orthodontist went to a town about 30 miles away once or twice a month to see patients there.
