The kids I saw in China wearing these were often lifted by their mothers by the thighs (just above the knee) from behind. This suspended them in a squatting position, and their genitalia would then be over the hole in the pants. Mother would hold them over a drain or the gutter while they did their business.
I guess one positive is they never get diaper rash.
