Y’know, I’m pretty sure that if someone grabbed me like that, I’d use my arms for something other than flexing. Especially since that particular form apparently allows for punching, given the way they’re windmilling at each other for the first few seconds. ---

I think y’all are missing something. The guy doing the penis grip is the victim of the hold. He does all of the signature gestures of the distressed wrestler. Slap the arm to indicate pain? Check. Grimace extraordinarily? Check. Drop to his knees? Check. I see it as the “attacker” grabbing the “defender’s” dink and then being overpowered by the awesomeness of the defender’s wiener. He then gets stuck in a painful magic-penis counter hold (notice he used his other hand to try and break the hold - a hold that got stronger the more the “defender” flexed.) From there the attacker becomes weaker and weaker until finally the attacker wallops him with a badly acted shot to the back of the head. ---
