My question is: how many spaces are allotted on the paper form for a FIRST name?

Um, I don’t have a registration form handy, but looking at the data record layout from ETS, first name is the third field in the data file, runs from position 24 to position 31 and, therefore, has 8 spaces allotted. (It’s also a string/alpha field.)
