I understand that when the character first appeared, she was unable to speak, or even have the concept of language. She first worked with Barbara Gordon. Did Barbara name her? How was the name ‘Cassie’ decided for her? Was that what her father called her, and if so, how did she communicate this with the rest of the Bat-family?

I’m not sure, but I think Cassandra’s name first came up in Batman: No Man’s Land, though I don’t recall how, and under what circumstances. More than likely it was in the sub-arc where David Cain comes to post-quake Gotham, on a contract to kill Commissioner Gordon.
It would have been in the No Man’s Land arc. Cassie’s name was established by Batgirl #1, which is when I started reading. (That was a great book under Pucket and Scott, BTW.)
So… Cain revealed this to folks before they started calling her Cassie? I guess what I’m asking is, when was it revealed that was her name, and who started calling her that?
I haven’t found it yet, but it didn’t happen in NML. No one called her by name in NML, and in Batgirl #1 Oracle puts “name unknown” when making up Batgirl’s file.
In Batgirl #12, Batgirl’s password to Oracle’s computer is “Cassandra”, which is the first appearance of the name I could find. Batgirl #17 is the first time someone (Oracle) addresses her by that name. Batgirl learns language in Batgirl #4, so I can only assume that she told them her name off-camera some point after that off-camera.
I’m pretty sure I knew her name before BG #12. My memories are well faded, but I believe Dick mentioned the name at some earlier stage (in a different book) while reaming out Bruce for something or other.
