Depends upon the owner/managers philosophy. Why would you think that there is a one size fits all answer? The lesbians I know do not necessarily hate all men, they just don’t want to have sex with them.
There used to be a (major) lesbian bar in our area called Club Metro. They had a mix of men, women, and trans as well.
I don’t know enough about other areas to say definitively, but from what I’ve seen, the proportion of lesbian bars is quite small compared to gay bars and even GLBTQ bars.
I (mistakenly) went it one once but had a great time. That one was in Boston and had male bouncers. I am not sure why they let me in because they turned away almost all the other guys. The only odd thing was the bathrooms. They had a men’s restroom but the butch lesbians used it freely as well. A couple were surprised when they walked in while I standing at the urinal. One yelled out “Hey, there is a real one in here!” but still came in anyway.
Varies. It’s very common to have female bouncers at the lesbian clubs I’ve been to in London, but not unheard of to have men.

maybe they only had room for one. In the US, there is no such thing as a lesbian only bar, or female only or male only. As we know, it’s very hard to maintain single-sex private clubs, let alone places of public accomodation such as bars. In light of the potential for money damages or loss of license, it is not worth it for a bar to turn awayall members of one sex or the other.
Maybe they just wanted to Shag Nasty style one last time. :eek:
