Find a neurologist who is a specialist IN MIGRAINE. I suffered through several who had no clue about migraine, and it was frustrating and a waste of time and money. There are so many doctors who have no clue about migraine or other types of non-tension headaches.
Hereis a list of docs certified in headache medicine. My own doc is here, and I recognize other names that are active within the specialty (speakers at medical conferences about headache, published studies, etc.)

At least for me, it only makes them worse and I can’t thing it’d be different for women.
Other aches and pains are usually helped by some sexifying, but migraines hate sex.
I’ve had migraines so bad I can’t even move my head. I’ve had them so bad I’ve woken up from the pain and couldn’t get back to sleep. There have been times I’ve wanted to cry because it hurt so much. Earaches, chills, nausea, the sensitivity to light and sound (not to mention smells), you name it. I sympathize with your wife.
I have found that ginger ale helps somewhat. It doesn’t cure a migraine, but it helps with the nausea and it even dulls the pain just enough that I’m not lying there wimpering. It has to have real ginger in it, though. Just a tip – it’s worth a shot.
Migraines are not caused by vasodilation, but vasodilation does often happen in a migraine. It thus can contribute to the symptoms. In which case, vasoconstriction can still help.
I’m also pretty sure that, with migraine, the relief methods were reported first, and then the reasons they might work were then surmised. Orgasm also releases endorphins, which are natural pain relievers. Even if the vasoconstriction doesn’t help, it doesn’t mean sex cannot.
But, of course, it can also trigger. Just like caffeine and brain freeze, or hot or cold compresses. Heck, it wouldn’t surprise me if triptans or other drugs also might trigger in some people. It makes me wonder if migraine is not one syndrome but several with similar symptoms.
When I used to get migraines, sex did nothing to help other than to take my mine off of it during the act. I’ve had them so bad that I could not do anything at all except sit in a chair in a dark room. Laying down made it worse I think, light made it worse, sounds made it worse…terrible.
Luckily I got that taken care of and never get headaches anymore.
Mine like good drugs, dark rooms, very soft classical chamber music, cool compresses and for me to be left the fuck alone. My migraines have absolutely no interest in sex …
