Has there ever been an instance in SDMB history where one moderator had to moderate another moderator? Have there ever been moderator feuds? Since the mods here are also quite active in the discussions, I can’t imagine it would be out of the question.
Not since I joined, but yes, it has happened. I don’t know too many of the details beyond that.
The whole melin episode comes to mind.
Moderator meltdowns are exceedingly rare but not unknown. Any warnings would have been private, I suspect, although the aftermath has been known to spill over on to the forums.

I was once one of several people he (temporarily?) banned for politicking in GQ; I don’t remember if any of the others came back or not, or if they did, they probably changed their names like I did. But given that his action was more or less justified even I wouldn’t call it a meltdown.
Of course, sometime after that he did go from Moderator to Member, but I don’t remember how or why that happened. It sometimes does happen that a Moderator just decides he or she doesn’t want to do it anymore and goes back to being a Member.
And then finally manhattan just left entirely, unable to put up any more with the board’s leftist slant.
Situations of moderator misbehaviors are handled in different ways, depending on the situation. Most common are things that wouldn’t merit a warning if done by a poster, so it’s handled quietly and behind the scenes. There have been perhaps four or five worse situations over the last, what, ten years? and I don’t want to bother to enumerate them. The last one was probably four or five years ago, and I don’t see any reason to scatch at scabs. (You’d be mightily pissed at me, with good reason, if I brought up offenses of some current poster from five years ago.)
I seem to remember a time when Dex said a few things that Euty (a mod at the time) took offense over about his SO at the time (the late Persephone).
Nothing serious, just an honest reaction (from Dex) over something she had posted that was, in fact, eyebrow-raising.
Jesus. Mine would fill a book. I mean, I’ve never intentionally broken a rule, and don’t even recall that I’ve ever gotten a warning.
There was that mess with the misunderstanding about my GP’s opinion of my chances of making it more than a couple years. But that was a sudden suspension, without warning of any kind. I did get an email, but was told that since I didn’t respond to it, I was being suspended.
It so happens that I didn’t (and still don’t) check my email every five minutes. Especially the Yahoo account. I responded the next day, and it wasn’t considered fast enough even though the person who emailed me infamously and repeatedly has apologized for not getting to emails for days. It smacked mightily of hypocrisy.
But that was long ago, and in a galaxy far away.

Just after 9/11, some obvious troll made some comment about a news story about a piece of wreckage shaped like a cross and how some of the rescue workers felt comforted by it and said that it should be torn down and that the rescue workers were lazy slobs and glory hunters (again, IIRC). Manny came back with a series of dead-on devastating insults (something about taking what the troll said and garbling it so it sounded like the troll wanted to “fuck his dead grandma”). Somewhere along the line, I think Manny was warned for it. (I don’t remember the details well enough, but I remember thinking it was bogus–100% on Manny’s side of that one)
There’s also Nickrz(sp?) the FORGOTTEN MOD!
He was a mod prior to my time and from the little I’ve been able to gather, he was a good poster and a bad mod–I never saw any examples of his modding (I think they disappeared in varying board upgrades) but third-hand comments suggest that he was totally over-the-top. He was retired (fired? asked to resign? I’ve never been sure) as a mod and has appeared several times since with varying degrees of coherence. (Last time I noticed him, he was throwing a huge shit-fit when the SDSAB tags were being removed from inactive members and his got taken away.)
