It occurs to me you might be thinking of Raging Bull (1980).

DeNiro really does appear to age, but not entirely through makeup. He gained something like 60 pounds to play the role of Jake LaMotta.
As for his marriage … IIRC, he knocks his pregnant wife down and kicks her until she aborts, all because he doesn’t like the way she cooked his steak.
Aside from the boxing scenes, which are brutal, the movie is memorable for being shot in B&W.

I’m pretty sure it was Goodfellas. I was surprised to look it up and find it was 1990; I had thought it was a few years earlier. But that’s still 27 years ago. And I’ll be damned if I can explain why I remembered which parts. I could guess but I think it would take a shrink. One thing I remember vividly is thinking that the makeup people did an amazing job of aging deNiro.

I think I’ll get it on DVD and watch it again (I have Netflix DVD plus streaming, but like every other movie I want to watch it’s not available on streaming).
But Henry & Karen didn’t really grow apart during the movie. She threatened to kill him when she found out he was cheating (again), but she was with him to the very end - she was with him when he was talking to the federal prosecutor about witness protection, claiming Henry couldn’t go anywhere too cold because he’s bronchial.
