How to punctuate zip code in text?

Hi?

If I’m writing a legal posting that has an address in prose, how do I punctuate the zip code.

For example:

An application of Bob Smith, 123 Fake Street, Springfield, QY, 12345, to…

Do I just put in a comma like that?

Yeah, it’s probably a really dumb question, but I’m a dumb guy!

Commas are used in addresses either to indicate a new line, or to separate like from like (alpha from alpha, number from number). No comma is necessary between the state and the ZIP code.

No comma, according to the Chicago Manual of Style.
