How's the accident/mistake record with the old Ed Sullivan show and the acrobats?

I’m watching the Ed Sullivan show on PBS, and saw my first mistake with one of the acrobats: He was spinning a top and doing tricks with it, dropping the top (but recovering nicely) a couple of times.

Does anyone remember seeing any other mistakes or accidents or otherwise bloopers on the show, involving but not limited to the acrobats? The whole idea of mistakes on this show is new to me; I never saw any flaws before this one.

Key for people to remember is the program was done live, and, if I remember correctly, an hour long. I vaugely remember one show that opened with a trapeze act, and early in the routine someone missed a catch and fell into the safety net, which ended the act. At the time I thought they might give them another chance later in the show, but they never reappeared.

I saw this on the Best of Ed Sullivan not long ago. The camera pulled away from the stage, so I had no idea whether it was blanks or live ammo being fired.
