Mine come from my contacts list so last names are attached. However on my speed dial list it’s only first names of my sons and their wives. For some reason I have a lot of friends named Chris so those all have last names. In one case both the husband and wife are named Chris so they are there under Christine and Christopher so I don’t get confused. Also more than one of my sons have more than one phone (landline, work, cell) as do some of the Chrises*, so there’s that, too.
Also my sons’ cells all follow the cartoon rule. Area code=where they lived in 2005. Got both coasts covered even though they’re all back in Denver now. So I could figure it out that way if I somehow lost the name on my speed dial. Which could happen.
I only have a my wife’s immediate family members in my cell phone since I never dial directly for overseas calls. That’s all Skype. Almost everyone in Taiwan uses LINE app for messaging and audio or video calls and the default setting is to accept their name as they have it.
My sons have a different last name than me. They are listed as - First Last (son). That way, if anything happens to me and people are searching my phone for someone to contact, they will know right away who to call.

Nope, just first names. Everyone gets just a first name unless I call them by a title like “Rabbi” or “Mom” (my address book is short enough that I know full well who everyone is).
On my older phones, where the limited number of buttons made typing annoying, names were as perfunctory as possible, and family members were often “mom”, “bro”, etc. With smart phones though, everything is synced to the contacts list which itself is linked to various cloud accounts, so it’s a bit more of a comprehensive database, whether I put in the effort to make it so or not.
No names for anyone, first or last. When my wife calls her picture comes up. Plus I know her number. Everyone else, and I mean everyone else, goes to voicemail. If it’s important, they’ll leave a message.
Yeah, my phone is just a flip phone, and the names I give people are often short. For addresses, I usually write those down in a paper notebook. If I had a smart phone I’d probably keep more info on it.
No last names for anyone (Why would I? Note I’m not using my phone for work/business purposes) - last name initials as needed to distinguish between people with the same first name - I’m 48.
I mostly keep to the following formats:
Friends and family are first name last name. The only exception is my wife, who is ICE - fn ln.
Coworkers I regularly deal with are fn ln W (work phone) and/or fn ln P (personal phone.) Those I only occasionally deal with are fn - division or facility name e.g.Tom - IT or Bob - Miami.
Finally there’s Company Name, for when I have to interact with a company but don’t have a particular contact.

Slight hijack. My phone allows you to put a couple lines of text on the lock screen. Even locked, it shows the time and ICE - Spouse with her phone number. That way, even if I just lose my phone, they have a contact number.
ICE for my partner, ICE BROTHER is second choice. It was a sad day when I had to remove ICE MOM due to Alzheimer’s. First names the rest, but tenants with the apt. # added.
