It would be really helpful to consult such a thing instead of trying to Google “arm pain” or whatever. I think I’ve seen a sort of book like that, but it dealt more with emergency situations than random questions.
You know my health provider (one of the Group medical outfit) sent me a book like that, with different types of complaints and what to symptoms to look out for. Basically a “When Should I Go to the Doctor?” type of reference book. I can’t see it on their website though so it might be members only. Are you a member of a healthcare association that might have something like that?
One of the major problems with this idea is that typical symptoms, from headaches to diarrhea, may have hundreds of causes. It is seldom possible to narrow the particular cause down in the fashion you want. That’s why doctors need so many years of schooling and on-the-job training. Putting such a simplified version of this out there could be incredibly dangerous.

As Exapno said, every minor symptom is also the symptom of some fatal disease. Real doctors know that headaches aren’t usually tumors.
