Shagnasty is right about forklift deliveries. In the factory where I worked, there were a few examples of 2nd story doors for just that purpose. The “mother in law door” is double, and just right for pallet delivery.

As for all the stairways to nowhere, that’s not that unusual in renovations. There once was a door there, and the owner wanted a bookshelf.

The wall built around half a toilet seems unlikely. However, I’ve seen something nearly that crazy. A factory restroom where I worked was being refurbished, and the new stainless partitions were measured wrong or something. The stall walls were OK for the first few toilets, but the right-hand walls got closer to the toilets as the row went on. The last one was smack dab touching the toilet. It was like that for a week before a new set was installed.
