There certainly appears to be a serious “dimple” to the lower-left of the actual navel.

The image in question is around 4:24. Immediately before that, around 4:18, there is no such dimple in the view from the other side. I therefore conclude it’s an artifact of the CGI. Maybe they had some piece of equipment or model pushing into his stomach, and they digitally removed the equipment but not the dimple.
Or maybe this was many takes in, and in one of the earlier takes, the bug burrowed a hole in the wrong place rather than going through his belly button. Whoops!
One of my aunts has a second belly-button, from an accident with a fishhook.

I have a scar just like that, from having a feeding tube inserted during radiation treatment. Perhaps Keanu had to have a tube for some reason.
It looks too low to be a feeding tube scar, but it might be from a surgical drain.
From an Esquire interview, in relation to his habit of riding motorcycles:

The accident left a thick scar up his abdomen and necessitated the removal of his spleen. Since then there have been other surgeries, fake teeth, veneers. Some broken ribs. "
If anything, they probably CGI’d scars out rather than in, and were inconsistent.
