Looking for an example 1099-R form for an IRA to Roth IRA conversion

I’ll omit the long backstory (but can go into it later if anyone is interested). The short version: I’m trying to generate a certain amount of taxable income by converting some IRA money into a Roth IRA. This type of conversion is reported to the taxpayer on a form 1099-R.

I’m trying to use a version of TurboTax 2018 to figure out how much I need to convert. I’m finding that small changes on the values in the 1099-R make outsize changes in the taxable income created.

So I’m looking for a recent sample 1099-R of an IRA to Roth IRA conversion. Does anyone have one? Specifically, I’m looking for:

  • The values in boxes 1 - 15
  • The codes in box 7
  • Whether the IRA/SEP/SIMPLE box was checked.

Please omit any identifying information. I’m just looking for something I can use as a model for what values I should enter into TurboTax.
