Use a site like this, plug in your state, print it out and you sign it, following your state’s dictates, like whether it needs to be signed in of a notary, etc. My bank will notarize things for me for free. Your treatment facility may have a notary easily available to you also.

Sorry about your falls. Definitely make sure you’ve got a way to immediately contact people when that happens. It sounds like you’ve got it covered, but you can never be too careful.
Alas, no rest. Setting up a medical alert pendant with my security company (for the falls), dealing with appointment check-ins. Just about out of spoons. 
You numb yourself for chemo? Where I go they do it for you. I opt for the cold spray.
Do you have a cane or walker? A cane has saved me numerous times.
She did mention a rollator, which is a sort of walker thing.
I missed that.
Prescription strength lidocaine. I put it on an hour before.

Ports are awesome, aren’t they? I’m a hard stick.and they take away so much stress.
They can be. I have a friend whose ports often get infected. May this not be a problem for you, Morgyn.
So far no worries, but i tend to leave the dressing on. I’ll ask them to keep an eye on it.
In other news, I can now TASTE the gel casing on pills. It’s FOUL. At least it’ll be over soon, as the antibiotic seems to have done its job. (And yes, I’m taking the full course. My folks did not raise idiots.)
Meantime, I’m also having a few bouts of vomiting. No fun, especially as it tends (so far) to happen at 3 in the morning.
It is with great sadness that I announce Morgan’s passing on 6/18/2023. Another friend and I were lucky enough to be with and holding her hands during her last breaths. Rest in peace, my love. I miss you already.
