Another vote for Irréversible. It was just so drawn out and brutal, I’m glad I saw it alone (a friend of mine saw it in film class and several people left the room during said scene. Some out of pure discomfort, I’m sure, but one of them was a friend who’d been assaulted on campus).
Also, the rape scene from The Hills Have Eyes remake (haven’t seen the sequel, never plan to). I’m used to metaphor in my horror– big knives and all that. Watching a mutant rape that girl from Lost in a generally shitty movie was not my cup of tea.

Yeah, that one was …intense. To say the least. It’s the way the guy keeps laughing as he gets hit and you see, hit by hit, his facial features disappear until his face is completely caved in. I almost couldn’t believe what I was seeing when I watched it.
The opening scene in Gangs of New York had me cringing and hiding my eyes. . .wicked makeshift weaponry! I finally had to get up and leave the room until it was over.
The curb scene in Generation X practically did me in!
Man Bites Dog (C’est arrivé près de chez vous), definitely…
Kill Bill 1& 2 also deserve a mention. [ETA: Tarantino’s films, for some reason, always rank up there, for me…]

Speaking of atrocities—Story of Ricky-Oh and Guinea Pig 2: Flower of Flesh and Blood are another couple of good ones.* The former features, well…this scene (probably NSFW), though the latter is a simulated snuff film (Only one actual onscreen death, though…stretched over an hour of drugged vivisection. Amazing what you can do with latex on a limited budget, really).
*And Japanese produced, to boot. Hmmm…
Eastern Promises is pretty brutal. The “sauna scene”, the murder at the beginning, cutting up the corpse :eek:

As far as the atmospheric thing…yes, exactly! I don’t even remember much about the actual things that happen in the movie, I just remember it freaked me out, and I’m pretty hard to freak out.
“Hostel” took so fucking long to get where it was going I didn’t give a shit by the time I got there. (See what I mean by hard to freak out?)

I agree. The scene really bothered me. It was too real.
In Casino, you watch someone’s head being crushed in a vise, while the guy is still awake and alert. I’m not sure if that’s the most violent scene ever, but it sure stuck with me.
