Lady Viola meets Lord Wessex. She is dressed like a lady but is still wearing her false mustache. The nurse covertly tears the mustache off so that Wessex doesn’t notice. Something about it is just… off. Like the mustache is already off, and the nurse is just miming it, and it’s out of sync with the tearing sound.
I can’t find a clip to look at it more closely.
In the John Wayne-directed The Green Berets, we see the sun set over the ocean. Given the geography of the setting, this should be impossible.

In The Green Berets, the rifle that John Wayne picks up and smashes against a tree in a fit of pique is clearly one of the plastic Mattel M-16s that were being marketed at the time (I had one, and it was really cool).
In the original Willy Wonka, when Gene Wilder is strolling through his chocolate room singing about imagination, he skewers a giant mushroom or something similar with his walking stick. You can see holes from several previous takes in the bottom of the mushroom cap.
Well, since you beat me to the punch, or lack thereof, a sort-of goof in godfather 2 is when Michael is courting Appolonia and we keep seeing the same guy in a red shirt either walking by or riding a bike in the background when they’re in the village.
Oops now it’s my turn to goof. That would be the original Godfather, not Godfather 2.
In this scene from North by Northwest, there is a boy in the background who sticks his fingers in his ears before Eva Marie Saint shoots Cary Grant. I should note that I probably read about the scene in an earlier thread on the SDMB.
The movie ‘Twister’ has so many editing goofs that a single post would only need to say
“Twister”
Watch any movie that involves skidding a car or motorcycle to a stop. You’ll most likely see several sets of skid marks from previous takes.
In The Untouchables, when Ness is at Malone’s house and they’re talking in the living room, the shots of Connery show the top button on his shirt alternately buttoned, then unbuttoned, then buttoned, then unbuttoned…
The shots of him are fairly close, like mid-chest and head. It’s ridiculously obvious. It’s almost as if the scene was an outtake from a spoof and they were doing it on purpose. DePalma must have been distracted.
In the film North by Northwest,at Mount Rushmore, in the restaurant, there is a kid who knows there is going to be a gun shot, he covers his ears before the gunfire.
A pretty famous one :
The Matrix - when Neo and the agent are on the rooftop having a gunfight, the agent shoots at Neo and he ducks backwards in slow motion. As the camera pans around, Neo’s two pistols are nowhere to be found, but when the camera catches up you see the guns right next to Neo’s body on the ground :
When I first saw Apollo 13, on a VHS tape, I noticed that Jim Lovell’s daughter is waving around a copy of the Let It Be LP in the scene where she’s upset over the news that the Beatles have broken up. That album wasn’t released until a month after the breakup announcement. The album cover is not visible on the DVD release, however, so either the scene was re-edited or the album is stripped out of frame in the widescreen format.
Apparently the train sped up while Spiderman was slowed by air drag. Um, yeah, that’s the explanation.
Similarly, the couple going into Jimmy Wah’s right before the explosion in Good Morning, Vietnam.
In The Man With The Golden Gun Bond is fighting with a baddie and at one point they bump a mirror, twisting it so that you can see half the camera crew. Clip here.
I’m not too sure if this is a goof or just bad story-telling, but in Pearl Harbor, bored American Ben Affleck joins the RAF as some sort of foreign volunteer, thereby leading to a tearful goodbye sequence as the England-bound flyboy boards a … train … for the trans-Atlantic journey.
Whatever it was, goof or bad plotting, it was a truly WTF moment.
