Need poem from 'Hamburger Hill'

My dad watched the movie ‘Hamburger Hill’ today on TV and asked if I could find a copy of the poem that’s shown at the end. I have seen a bunch of reviews online that mention the poem, but not the poem itself.

Probably against board rules to post the whole thing here, but if someone has the title and author, or can send me a link to the text, I’d appreciate it.
