Please help with a Spanish question!

How do you say “Exactly how is your name written on your Social Security card?” I am trying to find out how my nanny’s name appears on SSA records (it has to be exact when I submit the info), and her English is not great when it comes to complicated thoughts or phrases.

I don’t need pronunciation, just the phrase!

I couldn’t remember all the words, so I looked it up on Babelfish. It’s a literal translation, but it looks right.

Shouldn’t it be: “…como se escribe su nombre…”? Reflexive verbs and all.

On a related note, I’ve found that simplicity is the better part of valor when it comes to speaking to someone in a language in which you’re not fluent. That said, may I suggest simply asking “May I see you Social Security Card, please?” “¿Puedo ver su tarjeta de la Seguridad Social, por favor?”

“Exactamente cómo esta su nombre escrito en su tarjeta de la Seguridad Social” would be the most accurate way to say it IMHO (spanish is my first language).
