Seeking General Info on Calf Muscle Strains

Not seeking a diagnosis, I’m rather looking for some background on calf strains in general. There are a number of good resources on the web, but they left me still with a couple of unanswered questions:

  1. I gather that surgery is rare (extremely rare?) for calf strains. What are the factors that a doctor considers when evaluating a calf strain for possible surgery? (I assume that surgery must somehow sew the torn fibers together.)

  2. Articles describe the calf muscle post-strain as possibly having bunched or knotted portions - I gather that this must be torn fibers that no longer form a nice smooth continuous bundle. Is this a correct understanding? Do these bunched fibers eventually re-attach and smooth out? Therefore, is it expected that there could be a distinct area of “divot” due to missing muscle, and will this eventually fill in
