If the North Korean regime actually does have suicidal tendencies, and if NK indeed has a usable missile-launched weapon, and if you live in a major metropolitan area in the western part of the USA, than yes, maybe you should be a bit worried.
I think the far greater danger is a US pre-emptive strike (whether nuclear or conventional) on NK. It seems the temptation to do so before NK has more than a few missile-launched weapons becomes increasingly likely. Or, perhaps some impromptu saber-rattling goes wrong, and one side, say, sinks one of the other’s naval vessels, resulting in escalating reprisals. In either case, I have little confidence that there are sufficient cooler heads on either side to prevail.

If NK launches a nuke against us or against Seoul, the only response is to flatten them into glass. Trump would do it, Hillary would have done it, probably even Sanders would have.
It won’t happen, since Kim’s back isn’t against the wall just yet. He is posturing, as is Trump. We are not quite at the MAD stage with North Korea, since they can’t destroy us, but we can destroy them, and we would.

The last thing most dictators-for-life want is total annihilation. I think this is all about bargaining chips, and totally isolating NK just backs them into a corner. If NK launches a nuke at us or one of our close allies their country will be wiped off the face of the earth in a few hours. Problem solved.

I think the ones who would need to be worried are South Korea and Japan. NK may have the range for a missile to reach the US, but the US still has the ability to shoot missiles down. If we got into an all-out nuclear war with say, Russia or China, there’d be too many missiles to shoot down, but NK only has a handful at most, which is exactly what our current missile defense systems are designed to counteract.
Dear dog, I hope you don’t put too much faith and credit into that defense system. It’s not reassuringly reliable despite the billions that have been poured into it. Our Defense Against ICBMs.
It’s basically been tested only in highly controlled environments and even then, effective only about half the time. As a citizen who lives in the west, I am not comforted.
