I guess it could be real. Maybe posing the cat to get the image right? Still seems kind of odd for coloration on a cat though.

I could probably give you a good idea if it’s photoshopped if I had access to a much larger high quality image. I looks like a good job if it is though.
At first glance, there’s a bit right between the “ears” of the cat marking that looks a little off; there’s no blending between the dark fur and the white fur. Like someone did a quick patch job to give an otherwise round spot ears so it looked like a cat head. I could be wrong though, the photo isn’t terribly high quality. (JPGs - disguising photo-fakery behind poor quality since 1992.)
Aside from that, the mark doesn’t look like it’s outside the realm of possibility. A friend of mine has a piebald tabby not unlike that, with a thin white question mark swirled through the dark fur on her back.
