Turn-On (1969), canceled after the first episode.
Cancelled, effectively, after the first half-episode. (Many affiliates switched to other programming after the first 20 minutes.) I haven’t seen it in its entirety but it’s on my list for my next visit to the Paley museum holdings.
Too many series to list for me, because while I watch almost no “TV” we do watch select shows and so far have done start to finish.

I’ve never missed an episode of True Blood, Mad Men, or Desperate Housewives. I am At Home every single Sunday and not to be disturbed.
Probably my favorite of all time, Northern Exposure. Even the dreary last season.
I missed ONE episode of Lost, I did record it on a VHS tape, but have no idea where that tape is down in the basement or if I even wrote what episode was on it.
Sometimes I watch re-runs of Everybody Loves Raymond, which I never watched when it was on the air, and there must have been dozens of episodes, and surely I think I’ll see an episode I’ve never seen. Doesn’t happen. They show the same ones over and over, if there are episodes I’ve missed, they just don’t show those.
I know I’ll see every episode of Downton Abbey in the future when I watch the boxed set(s). Same with Call the Midwife.
Even when I really like a show it’s rarely so rabidly that I’ll not miss a single episode. MASH, Cheers, Frasier and Seinfeld probably fell into that category.
However, since they’re relatively new I probably have caught all of Modern Family and Duck Dynasty.

The OP should really be modified to cover only those shows you’ve seen on broadcast/cable/satellite. Boxed sets shouldn’t count.
I think there should be a distinction between 1) Watching every episode of a series, in order, from first to last, and 2) Being certain to have seen every episode over the years in a random, non-intentional manner.

There was another show with a brief run I saw in its entirety in 1996. It was called “The Straight Dope”.
Hmmm. Well, I’ve got easy ones, like Terriers and Firefly. But then there are the long-arc series like Boardwalk Empire, Game of Thrones and Breaking Bad where I couldn’t imagine not seeing every single episode.
(I also admit, with perhaps some degree of shame, that I’m fairly sure I’ve seen every episode of Burn Notice. :o)

Why, when I was a kid, we only had three network channels and a local channel. Shows were only rerun during the summer.
No cable, no VCR, no recording of any kind, and y’had to watch the shows when they were aired or you just plain missed 'em!
