MacGyver never had his first name directly revealed. A lot of people just called him Mac.

Though near the end of the series, he meets an adult son he never knew about. The boy is called Sam. Eventually it is revealed to actually be his initials. His actual name is Sean Angus Malloy. He says his mom told him his middle name was his father’s first name.

Consequently, most of his friends and colleagues call him by his last name or simply “Mac.” His first name is finally canonically revealed in “Good Knight, MacGyver”, in which he learns of a 7th century man, Angus M’Iver, and admits that they share their first name; and repeated in the series finale, which introduces MacGyver’s son, whose middle name is Angus. It is also revealed on the packaging of the MacGyver DVD season sets.

Spock’s first name is Mister.

Duh.

Leonard Nemoy once theorized that Spock’s first name was actually Harold (as given to him by his mother, honoring his human half) and that Spock only told people it was too hard to pronounce because he was embarrassed. I happen to love this idea.

99 used the name “Susan Hilton” in one episode, but later told Max that WASN’T her real name.

Similarly, on Chuck, Chuck supposedly learns Sarah’s real name, but later it’s revealed (to the audience anyway) that this also was an alias.
