Or was it shot on film? If film, what format did TV use, 16mm, 35mm, or…?

It was shot on film; 35 mm, I believe. Episodes shipped to Canada (and presumably to other places as well) were printed on 16 mm film.
Everything had been transferred to videotape by the mid-80s, I should think (for the home market). I saw my first ***TOS ***video in '82 or '83.
