This one had me baffled for a second, because I’m so used to using Spanish in everyday conversation here. My boss and I have entire phone conversations in Spanish sometimes, and neither of us are native speakers.
Seconded. In this country (Israel) if you call anybody you work with Mr/Ms anything, you are probably snarking at them or at the very least joking with them. It could be First Name, or Last Name, or Nick Name, but nobody, ever, uses titles – work related or personal.
One of our previous big bosses was always Mr. LastName. Currently everyone is FirstName except for our medical director, who is Dr. LastName. There are lots of people who call him by his first name and he introduces himself as FirstName LastName, but I’m in the habit of calling him Dr. LastName and don’t feel the need to do otherwise.

What do I call my boss? Twunt, usually. Technically she is “Firstname” but most of the time she comes out with directives that just bring “twunt” to mind.
I’ve work in a pretty casual environment (college dining hall) and everyone I’ve run across goes by their first name, right up to our Manager.
Anyone my age is called by their first name and most older than me are called mr./mrs./ms. lastname.
So my boss and her husband are Mr. and Mrs. lastname or Boss/Chief/Fearless Leader and Mr. Lastname.

I call her by her first name.
Sometimes I also call her “Fearless Leader”
I called my last boss “Judge,” and the boss before that “Professor.”
My current boss, “pravnik.”
I call my boss by his first name. It’s standard practice here.
“Deb.”
(Or, when speaking to the kids, “Your mother.”)
To his face Firstname. Firstnames for everyone in the company actually. Though I often sign myself off with just my surname which is slightly strange as I’m female.
Behind his back Firstname Tom. Though only because there’s a few people who share his first name. And I should add that Tom is a job title (no we are not hookers).
First name. Everyone uses first names at my office, right up to the top levels of management.
In movies/TV, I’ve seen a lot of people call their bosses “boss.” I’ve never heard that in any of my jobs (except in jest).

…because she has a habit of handing out [del]orders[/del] direction to people who don’t actually work for her, and because every manager in the place (besides her) is an incompetent boob, as far as she is concerned.
But I actually kind of like her. She’s always been nice to the people who really do work for her. I suspect she’ll be fired within a year, though.
Both my immediate boss and my CEO are usually first name, although I’ve been know to call the immediate boss ‘Our great leader’, ‘media tart’ and ‘fuckwit’, both to his face and behind his back.
I don’t use any name. I get his attention and start talking. If he’s facing the other way, and an ahem doesn’t do it, I tap his shoulder.
Every place I’ve worked it’s been first name - but I’ve always worked in research or high tech type companies. Where I work now, a billion dollar company, the CEO always goes by first name.
And since I like my bosses, all the way up the line, no nasty names behind their backs.
My immediate superior (and everybody else in the company, in fact) is first name, or “boss” when we get antsy and we all start talking like we’re in a chain gang. The company president is “Sir,” or Mr. Lastname, or Lastname-san when talking about him.
