Mine is $12,500 per year. I pay half and my employer pays half. This is coverage for a family of four (2 adults and 2 children) and includes dental and vision coverage.
Can you clarify the question little? This sounds like a poll and should be in IMHO in that case. I work for a gigantic benefits outsourcing group and I have data on how much insurance costs for millions of individuals in many different large corporations. The overall cost for yours sounds about right if it is a family plan. However, your employer contribution sounds low so you are paying more yourself than most people with a comparable plan.
$2400/yr for a couple in mid 30s. $10 million in coverage, $2000 deductible. 100% paid by us as this is private coverage. Valid everywhere in the world except the USA.
$5700 a year for a family of four, no dental, no vision, no maternity, $2500 deductible, $5000 out of pocket maximum, $2 million limit (I think?), $500 annual well-child allowance (not nearly enough for the required infant immunizations in my state). Private policy PPO through Blue Cross Blue Shield.

£1871pa or $3743/year (roughly 8% of my salary). It covers all medical costs (including free prescriptions due to my medical condition), state pension contributions and unemployment insurance. It also subsidises dental treatments (max dentist’s bill will be ~£190).
It also covers me completely (with respects to emergency medical care) in 30 other countries.
I like my insurance company.

Single, no dependents. Can’t remember my deductible.
I pay 5% of my salary. Just like every other citizen in the country. Covers absolutely everything, zero deductible,… but I do have to pay about $15 a month for my meds.
(The money goes to the government Dept of Health, which then passes the money on to the private HMO’s).

I think I am paying US$1,300 or so per year for just myself with a US$10K annual deductible. My employer covers expenses here, so the main thing I wanted was worldwide coverage.
Self-employed, pay $3000/year for coverage that doesn’t pay a thing until we meet a $3K deductible. As we’re fairly healthy people, that’s a decent deal for us.
No dental or eye.
Based on previous experience, I can expect the cost to go up about 25% a year.

Mine’s “free” to me - I don’t pay any extra, my employer pays the old fashioned way. I’d have to pay if I had anybody else on my coverage, but I’m single and childless.
I don’t pay anything for mine, nice perk of working for an insurance company. No deductible, no cap on the coverage, $5 for doctor visit, no copay for hospital stays. I can cover my SO for nothing as well, but because I’m gay would have to pay federal tax on the street value of his portion.
For just my wife and I, I pay $54 per paycheck, or $1404 per year. We have a $150 deductible and $15 office visit copay. This covers medical, vision, and dental, and 100% of hospital stays are covered, once the deductible is met.
This insurance also covers 100% of fertility treatment, as well as 100% of birth and delivery costs at the hospital. This works out well since my wife and I have recently decided to have a baby, and things aren’t going as smoothly as we had hoped.

But, of course it’s buried somewhere in the ~ $30,000 odd taxes I pay.
Our plan costs about $4800 per year for two adults and two children. We have no doctor’s visit co-pay, and our prescription co-pay is $15 after an initial $50 deductible. We have sweet coverage - it helps that my husband works for the company who sells it, but that is the non-discounted price.
My employer pays the whole tab, about $15,600 per year for health insurance (employee, spouse, children).
