As for Bernie Madoff, people like him who are ultra-high-profile are usually placed in protective custody, regardless of what they did, because famous people are targeted in prisons along with police officers and child molesters.
Nobody. Staff doesn’t clean it and you’re always short on cleaning supplies. There is hardly toilet paper unless you buy from canteen. A prison toilet has no seat, made of cold metal and has very powerful flush, it doesn’t have a tank. the flush is like a shower you keep pressing the button while you’re going to keep smell down.
When you are in general population, there is shitter and pisser. Half toilets are used for pissing only to avoid splashes. local gang is making sure you don’t forget which one is which. black or latinos in general, and skinheads in protective custody. this answers both questions at once, most skinheads are in protective custody

in 14 years of overseeing medical care for over 23,000 inmates, I’ve referred two folks for circumcisions; both for phimosis, a medical condition necessitating it.
I’ve not sent anyone out for a bris. 
As far as the misfortunate Madoff is concerned:

There is the slight possibility, of course, that Mr. Madoff’s affidavits are not worth the paper they are written on.
