Los Angeles.
I have been to most if not all of the major cities East of the Mississippi, but only a handful West of the Mississippi. Detroit is one city that I know I have never visited.
I have never been to any major city in California or Arizona but I have been in both states. (Lake Tahoe and Hoover Dam). I have been to Vegas, Denver, Seattle, Dallas, and Houston.
Same here. The largest city I’ve been to (was born there) is Los Angeles, which ranks No. 2 on Wikipedia’s list. Haven’t been to No. 1 (New York), or No. 3 (Chicago) but have been to the other large cities in the West: San Diego, San Jose, and Phoenix.
Does spending a week in Long Beach count as having been to LA? If so, Houston. Of not, LA.
I’ve been to New York, LA, and Chicago, but not Houston. If I ever go to Houston, I will be sure to walk right.
I’ve never been to New York City (I drove from Virginia to New England and purposely drove around New York because no way was I going to drive into it).
I’ve also never been to Chicago or Dallas, though I did change planes in Dallas once.
Calgary and then Victoria for me.
Even though one of my best friends lives there, I’ve never been to LA. I hope to fix that within the next few years. I’ve also never been to Houston or Phoenix, but I’ve less incentive to fix those.
If you count “city” as within the outer bypass loop, it would be Phoenix, followed by Honolulu and Anchorage.
Hobart. Although I really only went to Perth as a child or, as an adult, passing through the airport on my way somewhere.
In the top 20 I’ve never been to Dallas/Ft Worth.
At the top of the list: I lived in or around NYC for 25 years, but have never been to LA.
In Canada, the largest city I have never been to is Laval.
Number 6, Málaga. The other one I’m missing in the first 30 is number 9, Las Palmas de Gran Canaria.
I’ve been to New York, and to the Los Angeles area (Irvine/Costa Mesa), but I haven’t been to Chicago yet, other than to switch planes in O’Hare.
I’ve been around, at least in the US. #50 Buffalo.

If I understand the question correctly, the largest city I’ve never been to is San Antonio, which is #7, since I’ve been to Phoenix, at #6. Never been to NY, LA, Chicago, Houston, or Philly.
How long a layover in an airport does it have to be to count? I’ve only been in the airport in Chicago on a long layover. After that it would be Phoenix.
I might be considered to be stretching things a bit, to claim Cardiff: have passed through by train / changed trains there a good number of times, and passed close by, by road heading further west; but to the best of my recollection, have never paused to explore the city.
Giles in his “15 biggest metro areas” thread on IMHO, would per his standard set there, disallow Cardiff for me. However – I enjoy, and have an interest in, travelling by rail: so if I’ve passed through a place on a train, I’m happy to count it. A couple of other towns on my “been-to” list, I have also only passed through by train.
