For quick pain I’d have to say acid on my clit ranks right up there. (VD issues, don’t ask)
For longer duration…I’d say it’s a tossup between bursting ovarian cyst, which someone else described, and earache. Oh wait…anal fissures are pretty nasty as well.

For all you kidney stone sufferers out there - my mother had a kidney stone about a centimeter in diameter. First the doctor tried t crush it, but it was too big and too solid. So then he decided to manually push it through the ureter. LOTS of pain. later, after a week my mother (a tough old broad who’s had broken limbs set without anesthesia because of drug allergies) is complaining that there’s too much pain. The doctor pooh-poohs her complaint, so she goes for a second opinion. It turns out her pushed the stone right through the wall of her kidney (leaving a hole the size of your pinky in it). Whoops! Let’s see…what else has she had? Cervical cancer (while carrying me - the reason I’m the last of only five kids), a car accident that smashed both legs and severed one foot, a mastectomy, an aorta replacement, gangrene, multiple drug and chemical allergies and the loss of her husband of almost 50 years.
BTW - my sisters have given birth to 8 children, collectively. All without any kind of drugs. They said it was no big deal. One left the hospital 6 hours after delivery, saying they wasn’t anything they could do for her that she couldn’t do for herself.
