Where can I find a copy of a Bush loyalty oath?

I’ve been hearing for some time now that the Bush campaign is requiring people attending rallies and whatnot to sign a loyalty oath stating that the signatory supports the reelection of George W. Bush and consents to the use of his name and image. However, I have not yet seen an actual copy of such an oath. What’s the straight dope? Are they real? Are they being used at all public functions? How long have they been in use? Is there anywhere I can go to see a scanned copy or a duplication?

An article referencing the original loyalty oath:

An endorsement form provided to the Journal by (Pamela) Random says: “I, (full name) … do herby (sic) endorse George W. Bush for reelection of the United States.” It later adds that, “In signing the above endorsement you are consenting to use and release of your name by Bush-Cheney as an endorser of President Bush.”

In a related vein, a US serviceman from Iraq notes that soldiers also had to fill out a loyalty questionaire before they could have Thanksgiving dinner with President Bush:

What did you think about President Bush’s Thanksgiving visit to Iraq?
I was there when President Bush came to the [Baghdad] airport. The day before, you had to fill out a questionnaire and answer questions, that would determine whether they would allow you in the room with the President.

What was on the questionnaire?
“Do you support the president?” … And if the answer was not a gung-ho, A-1, 100 percent yes, then you were not allowed into the cafeteria. You were not allowed to eat the Thanksgiving meal that day.
