If you’re counting clones, then you gotta count long-term parallel-world substitutions in which case Uncle Ben was alive for quite a while (6+ months?). There was a story arc in the later '90s (after the Spider-Clone stuff, before the JMS stuff) where an evil Uncle Ben from a parallel world ended up on ours. I have no idea how the story turned out as I’d long since quit the book, but he was in a number of issues I read at the comic shop. (I assume he was sent home or died horribly…or both, perhaps.

She must be dead again, though, since the “Death of the New Gods” bit that preceded Final Crisis.
And most likely reborn after FC, since Earth-51 was implied to become a new New Genesis/Apokalips.
Of course, there’s presumably a new version hanging around in the New 52.
