Nope. I don’t recall ever filling out a form and being asked my mother’s maiden name except as a security question. I’ve filled out a lot of forms that asked for my maiden name but essentially treated it as a type of alias or AKA, asking about any others at the same time. In fact, I always have to list my “married” name for this question, because although I myself have never used my husband’s last name , I’m not sure that no one have ever used it for me.
yes, it was a way for gay to protect their relationship & legal rights, back in the days before same-sex marriage.
